Guardian 00610 CB-1-W Weld on Anchor 

The Guardian Fall Protection CB-1-W Weld-On Anchor is 2.75" in diameter, and may be used in a variety of fall protection applications. It includes a forged steel D-ring and can withstand loads up to 5,000 pounds. This anchor is suitable for installation onto either vertical or horizontal steel structures.
